Sabbatical leave

I am going through a tough time with my physical and mental health.

I want to ask for Sabbatical and need suggestions on how to ask for sabbatical, what medical conditions they approve for sabbatical because I heard it's not easy to get sabbatical leaves approved. Whom to reach out first HR or Manager?

I don't want to resign.
