How referral works? Is it a good culture?

How the Referal works...actually speaking...isn't there be fair opportunity instead of referrals....because it feels like big favour is being taken and also favouritism etc comes to play.... and how it works in big four or big 50 firms..??

Is that managers time to time inform its team about referral drives ??
