I have never needed anything related a dental treatment more than once, when I had to get a procedure done, I got it done out of my own pocket.

Afaik, employers that offer dental insurance to their employees do so out of their own pockets. Insurance companies don't offer plans for dental insurance in India. The only way they do that is if the employer will reimburse the insurance company the entire sum.
