I joined the organization at A4 level in September 2024 and my first review is due this quarter. Can I expect both appraisal and promotion to A5? What rating is required to get a promotion? Are there any other factors which decide your promotion or will I remain at A4 with a 5-6% hike?

Most of the freshers do get promoted to A5, even if you've received succeeding (3) rating. But don't expect high hike with this promotion, realistically, if you receive succeeding then you can expect 5-6% hike
