Withdraw pf amount for personal reason by mentioning illness to family member

I have 1L change in pf account.For illness reason, I am eligible for 82K.So I want to withdraw entire 82K.suggest me the way. Note: I don't have any medical certificates. Do we need to medical documents of my family member?
