What does Tier 1, Tier 2, Tier 3 really mean?

See a lot of talk about Tier 1/Tier 2 etc

What do Tier 1, Tier 2, Tier 3 really mean? What are the origins of these titles. Is this something that the colleges started doing or this is something HRs started doing?
