Hospital registration certificate. Doctor registration certificate. Invoice. Receipt. Gravida status by hospital (search on google) Ultrasound report, other lab reports.

These documents I had provided while claiming the amount. Mine hospital is not included for cashless treatment.

Including prescription, room accommodation!!
