I want to get a Letter of recommendation from my manager. What are the steps? can I get it on a blank page and add Capgemini Logo or letter head on the top or how it should be done ?

Talk to your manager directly. Don't add logos or letterheads yourself; it needs to be official. If HR is unresponsive, escalate the issue. Public shaming on LinkedIn is a last resort.
