Trending @Accenture; What's PIP? What enables PIP?

I see very often L12, L11 folks here mentioning about being pushed to PIP. That's brings me to question what is PIP (apart from Full form) and why is it said that 99% lose their job when are are put to PIP?

PIP : PERFORMANCE IMPROVEMENT PLAN.

When an individual is not performing well and comes under the lower band. Then that person is put in to the PIP. During this period some tasks are assigned to the person and they have to complete it. Since the assigned tasks are generally difficult, so people won't able to complete them. Which leads to termination. If someone is able to complete the task and clear the PIP, and he is back on to the normal track.
