It's not like you are not eligible, there's no policy as such ...but if ur base pay is very less at ur level..then manager will consider u ...all depends on managers and the budget they are given..

Yes and no also. Depends on many factors like manager, project, current CTC

My friend was promoted in June cycle and got good hike in December cycle
