Trending @Infosys; Sabbatical - How to ask?

I'm working in Infy since last 3 years back to back in development project and now I'm exhausted. I don't want to work anymore anywhere. I want to take a good break of 3-4 months and then come back with more energy. My finances are sorted. I wanted to know how to ask for sabbatical? I don't have any medical reason to give excuse for. What reasons are god enough to get sabbatical? Let me know if anyone have taken it. Appreciate if you may share your experience if you have taken it earlier. 😊
