Rituals you follow for long meetings

Sometimes, when meeting with leadership and executives, too many points get discussed. How do you ensure that you have clear action items in the end? Is there a ritual that you follow?

I scribble lot of things in my notebook. And most of the times, while preparing minitues of meeting, I miss out on important stuff.
