Request for Relocation or Deputation Due to Health Issues

Hello everyone, I have severe anxiety and depression, and there have been many nights when I couldn't sleep properly. Because of this, I am struggling to work efficiently. I consulted a therapist, who analyzed my condition and provided a medical report. I am planning to submit this report to my manager and HR to request a relocation. If relocation is not possible, I would at least like to request a deputation until my health improves. What are the chances that they will consider my health issue serious enough to approve my request?
