Trending @Infosys; Refferal Question

I had referred one of friend to infosys she had cleared technical and managerial both and salary discussion also done and they said offer letter will be releases in two weeks and one fine day HR called and one more managerial round after they was informed that we have not approval from my managers so we are nor proceeding further with your profile… how it is possible i dont whats happening here … any idea about this process
