Referral form from manager for IIM B PGP interview

I have received the call from IIM Bangalore for their PGP program and as part of the application form I am required to get reference from my manager/team lead.

Can someone elaborate what all pointers should be covered in the reference letter? Will give me a sense of what to request my manager while asking him to fill the referral form.
