Question on background verification

In my BGV process it was found that I was a junior DS in one of the previous orgs. In my resume I mentioned it as just DS, without the 'junior'. The HR has informed this has come up as a red flag from the BGV company.

This is weird. Has this ever happened to anyone and will this lead to termination? If yes, do they also ask to pay back the salaries? I have already worked for 3 months here.
