Promotion is stuck

My promotion has been initiated last year. It has been approved by my lead and manager. But they are saying that it is pending with the HR. When I inquired about this, HR was saying that she didn't get any list from my team. What to do now? I saw it in the portal and the status says pending with HR.

Your name was not sent to leadership team from your manager. Hierarchy is manager-> leadership-> HR team.
