Recently my manager has agreed to make my transfer based on the project requirements, but she is not aware of the process, Can anybody guide me what steps she should be taking?

Ask your manager to work with the PMO team. They need to open an RRD in the destination location and then lock you to that RRD which will automatically initiate and OIT i.e Organization Initiated Transfer. You will get relocation expenses in such cases if you are eligible for it.
