Trending @Accenture; Probation clearing Chances

I joined an internal team in August, but my performance hasn't been good, and I accept my mistakes. Recently, my people lead mentioned on a catch-up call that my probation might be extended or delayed, but it wasn't clear. I'm unsure how to interpret this. No Senior Manager or member of management has discussed this with me yet. What typically happens in such cases? Do they usually inform you on the last day of probation that it's being extended? What are the chances of clearing probation after an extension? Any personal experiences or advice would be helpful.
