On which basis can a company put you on PIP?

Also who is responsible for assigning a PIP, manager or HR?

As per my knowledge performance basis they put on PIP and manager is responsible for that

Totally depends on manager, specifically the one who is handling you.
Another thing what I have heard so far is in yearly evaluation if you got below 3 that is 2 or 1 out of 5 mean there are high chances So adviced that you must put you progress updates in an attractive manner in I Count.
