Pf transfer to tcs trust from EPFO.

I've transferred my epf but its showing at the bottom of form that take a printout and duly sign it and submit it to tcs trust, but where i exactly need to submit is there any email, also i need to withdraw it for my wedding expenses. Please let me know the pricess if anyone had done it earlier.
