PF transfer query

I'm currently serving notice period at TCS. I received both PF undertaking (transfer) form and PF withdrawal form via TCS offboarding process mail. I wish to transfer my PF amount to new company, do I need to submit both PF undertaking form and withdrawal form or only PF undertaking form?

Also, I have only 2 years of experience and I'm not eligible for gratuity. Do i need to fill and submit gratuity form in my case?
