Packages at big product based companies

Was just wondering, whether the people who get 200-300% hikes at big product based companies ask for it, or the companies themselves offer them this much. If people ask it, doesn't the HR say that it's an unrealistic expectation?

If you get strong hire feedback in the Interviews, they will by default offer you max pay for that role.

It has to do with the quality they showcase, value they can add, and the limited availability of their skills. Without any of these, why would they even think of offering/expect to receive requests for such hikes.
