Notice period extension

I'm currently serving my notice period, with 60 days remaining. However, I've recently taken 5 days of approved medical leave due to health issues, including a diabetes diagnosis with sugar levels of 518. My manager is now requesting that I extend my notice period, but I'm not in a position to do so. In fact, I'm requesting an early release to focus on my health and restart my career. Working on the critical project has been causing me significant stress, which is exacerbating my condition. I requested consideration for an early release, given my health circumstances.
