Need advice regarding appraisal.

I was on LWP between 3rd June -20 june because of health issues. The very next day after my leave ended, I reported to RMG and after having multiple followups with RMG and HRs I got my access back of Ultimatix on 28 july. But the appraisal form last date was 15 july and I couldn't fill that time. I want to know if it is possible now.
