No documents needed. Talk to your manager on a call and then send a mail that as discussed i am taking leave for so and so days.

Actually I am unallocated from 3 or 4 days before that already had communicated with my previous team but I had make one mistake that I am going on leave from next week so I applied that today. Now what would I do as leave is not approved.
