As I have health issues reports are submitted to HR and HR responded to take relaxation for 1 month.

If I may take relaxation is it an effect on my salary?

Means loss of pay will be detected when I am leave

Yes, if you take a month off without paid leave, it will affect your salary. Loss of pay will be deducted. Talk to HR for specifics.
