Lerf process after maternity what are the docs required

Hi all, after maternity leave does Discharge Summary and birth certificate of the baby is mandatory? What the mandatory docs required with LERF? Does LERF can be applied only the date of the joining i.e) on the same day of joining only the system is allowing to fill the day. Anyone can clarify on it pls
