Download medi assist mobile app. The login with emp I'd ie 123456@infy
Then you will have pop up for corporate login
Following this you will option to raise claim and bank details.

Share ur meddi assist card (urs and ur wife). They will ask few more proof such as aadhar ,pan etc. Also u have to share the copy of company id card.

Then they will take care u will get all messages of the process
