Actually I contributed a lot to the project, worked more than 13hrs, attended 3-4 meetings everyday but was given needs improvement rating. There were no issues with me when I worked till late nights but issues started when I asked for release. Release was due to my health issues and also due to lack of team management (onsite lead) there were lot of last min req changes given which made us to rework till late nights and also had a toxic colleague who never missed a chance to steal my credits.

She said release won't be possible soon so I said will work till evenings. In release mail I mentioned 9 to 4:30 working hours for few weeks until my recovery. But I didn't work in those timings till date rather worked till 7 or 8pm sometimes more. Messaged my status to her/ in group before log off. She started complaining more since then. Complaining in general was normal as she scolds everyone on a daily basis. But she also targeted me saying that I'm not fit for any project and mocking every small things,also on my timings mentioned in release mail.

Even after all the work submissions affected my rating. She gave feedback as rework was due to my work and lack of team collaboration. I did not receive any appreciations,awards or any shift allowances. But all I got was complaints on my status updates given at end, me working till late nights due to lack of technical skills and said I haven't contributed anything. I can still see all my work in prod and QA. I don't deserve any of this. Some managers are really worst in Infosys just want to end someone's career because they didn't like them or not able to work late nights.

Atleast I got released from the project. But she has warned me if I go to HR then it will affect my attitude which maybe due to that release mail timings and all the complaints list ready to show them. Should I still report all this to HR. What do you think? Will it make any difference on my rating or my career here?

No one will listen to you. You got Need Improvement, soon you will be given PIP and then you know what will happen.

Ask them data on why you have been given this rating

You are part of a big organisation where the manager is your supreme leader Aladdin (The Dictator movie reference). If your manager has the audacity to be warned you going to HR, just for re-evaluation

I would request you to move out of the company, and in the process escalate these behaviours. If you're planning to stay, please collect these proofs and keep them to the last few months of your stint at Infosys - would have better peace of mind this way

You can always go to manager and hr for re evaluation

Similar situation for my friend, her manager said, " I will show you who I am", for going and informing the group project manager about her situation. Luckily, there is already a cold war going in between Pm and Gpm, so he helped her get released.But she was given a very bad rating saying she was not a team player and all.
