Include probation exp or not?

It's been 2 months since I joined this company and they have a few concerns.
- Manager's expectations weren't met.
- I hadn't told them about my chronic(1y) health condition earlier (hid it coz I thought it can act as a red flag for any employer)
- I couldn't work from the office due to the same (work mode is hybrid)
Yes it took some more time for me to understand and implement things as compared to other new joinees. I'm trying to do better each day but it seems I've already lost it and they told me they might not make me permanent/cancel contract as the probation will end in a few weeks.
So I think it's better for me to try to save my job as well as keep looking for a new one if things don't work out. And preferably a remote job as I need to prioritise health, diet and other help from family which I might not have if I relocate to any location away from home.
