I will tell you what happened with one of my colleague he was a consultant
Since the start of this year, my colleague was coming to the office only 5–6 days a month instead of the required 12. At the end of August, our project ended and we were on the bench. Around that time, he met with an accident and had his leg in a plaster for two months, so he couldn’t go to the office. He raised a BTO exemption request, which got approved up to the director level, but a senior director later rejected it due to some issue with the dates, which turned out to be a bto exemption system error. The senior director asked him to raise a ticket with HR, and he did, but the whole thing dragged on for two months over emails.

After recovering, when he tried to return to the office, he got a disciplinary committee mail for having 0 office days in the last two months. In the meeting, he explained about his accident and medical condition. The committee said "okay we understood because of leg fracture you didn't come to office from last two months but why you have been coming only 5–6 days a month since the start of the year?" He couldn’t give a strong answer, and after six days later, he got a termination mail with one month’s salary.
