How to avail leaves during Notice Period?

I want to avail 5 days of leaves during notice period for the vacation on my wedding anniversary. How can I take leaves? Any suggestions?

Only sick leave is allowed on notice period. And for more than 3 days sick leave you will need medical certificate. If you have any known doctor etc seek his help and submit medical document.
