Trending @Infosys; HelloDoctor portal documents to upload

Hi, for critical medicine reason for work from home, its mentioned that along with your medical documents/reports you need to upload Infosys Format Medical Form, But i am not able to find that medical form format anywhere? Does anyone know about it?
