Health issues need transfer in tcs

Hi, I was working on a project for 2 years and for the past 4-5 months I was suffering from health issues and I'm working in a different city. I need to transfer to my hometown, there is also a tcs office present. I was taking WFH but still they are asking me to report to the office.

What is the procedure to get transferred to different location in tcs?
