Health Insurance Deducted Thrice this time

Health insurance premium was deducted 1500 this which was 540 last year . Even I haven’t changed anything in HIS . Why this was thrice as compare to last year .

It happened with me as well I changed from gold to gold plus, and now deducted 6100 rs from my salary Will 6100 deduct every month or just once or twice? Any idea please help
