Trending @Accenture; Gratuity doubt

Hi Guys, I am leaving Accenture and I need to fill the form 1 for gratuity.I am getting basic pay + hra+ personal allowance in my monthly salary.So do I need to only enter the basic pay from this breakup or I have to fill in my full monthly salary amount(badicpay+hra+personal allowance) itself.? Appreciate any help here.Thanks in advance.
