Getting ill with anxiety

Ipf - 4.3, C3A - annual appraisal. Last band C, this year also hinted at a C band. Exp. - 12 yrs. RTO compliant with 100%, 5-7 hours bracket average. No issues with my boss in any argument ever etc. What to do?
