Fresh H1B lottery OR Cap Exemption?

I have used my petition for 5 years in USA and have remaining period of 1 year. Is it wise to ask for cap exemption or fresh H1B lottery to get another 6 years?

I believe you need to start from beginning which is lottery or else get i140 approved before your remaining 1 year visa expires.
