Does it matters meeting deadlines, when everyone is at fault

This is going to be a broader post directed to a lot of stakeholders, a team will be mostly comprising of FE, BE, product.

When the team misses the deadline because of multiple reasons(Evolving product requirement docs), BE -- Wrong estimations or didn't account for changes like the code has to support landing on moon and taking those photographs. FE -- just assumed simple CRUD operation, didn't account for complexity and multiple API call involved.

How is it dealt with? who takes the fall and how the situation generally gets salvaged? I think this happens quite often.
