Mediclaim is valid for a year . You can avail till the end of the year .
Your contract is with the insurance company so that will be valid even after you leave the organisation

I think if you have taken a voluntary top up for self or family then that will stay valid however if it’s the organisational one for self then it’s validity is till your last working day
