Trending @Accenture; Base location change -recent work from office mandate.Please help me

I completed nearly 1 year in accenture and the base location is kolkata.I asked my manager to change my base location so she agreed on my teams call and she provided me a exception tool link where it can only stop me for maximum 90 days.So after the call and checking the exception tool mentioned her that its only upto 90 days so she said okay first you send that exception tool request and lets see the further action if anything comes.But how to change my location.The above procedure is not changing my base location its just stoping me for 90 days.so anyone please help me what to do at this situation i need to change my base location and i have a genuine reason of family health issue.So i need yo know the procedure of changing my base location and who will initiate the request.
