Appriasal process

Last year I was on bench from April to July. In August I have got allocation in another project. Now in speed with respect to self evaluation, I have two appraisals. First is from April to July and second is August to March. For first appraisal from April to July, what can I do? Who will add goals and will give review and ratings?
