Appraisal

I have initiated appraisal in June when I was in different project ,and got allocation for few days .but from August I have joined new project but in speed I am not able to change swon details. So who will give me ratings and band?

Your immediate supervisor from the June project will handle your ratings. If there are issues with SWON details, talk to HR or your current project manager.
